Fay Spain was born Lona Fay Spain on October 6, 1932, in Phoenix, Arizona. She came from a family that included her parents, Robert C. Spain and Arminta Frances Cochran Grant. Throughout her life, she was married four times, with her first husband being actor John Falvo, with whom she had a son, Jock, born in 1954. Fay won a scholarship to the University of Washington at the age of 16, but she left to pursue an acting career in New York.

Fay began her career with summer stock and small television roles, including an appearance on the show You Bet Your Life. By the late 1950s, she starred in popular TV shows and films, gaining fame for her roles in Gunsmoke, Perry Mason, and other productions.
